MANAGER REHAB SERVICES
Northwestern Lake Forest Hospital

In this role you will ensue and maintain quality patient care while coordinating Rehabilitation Services for patient care activities for the Extended Care Facility in Lake Forest. Candidates must have at least three years of related work experience, demonstrating a working knowledge of this position's responsibilities and methods.

Requirements:

• Bachelor's degree or equivalent.


• Thorough knowledge of the theories, principles and practices in a professional discipline, specialized or occupational field.


• A current Illinois license in Physical or Occupational Therapy.


• Knowledge of MDS system and experience in a SNF/ECF preferred.
