Physical Therapist
Northwestern Lake Forest Hospital

In this role you will evaluate patients and gather appropriate information to determine accurate clinical diagnoses and treatment plans. You will establish appropriate goals based upon functional limitations, diagnosis and patient's home setting. This role requires you to communicate those treatment programs to the patient, family and other interdisciplinary personnel; you will also provide emotional support as needed for the patient and the patient's family. You will coordinate and implement these treatment plans.

Requirements:

• B.S. in Physical Therapy.


• Current PT IL license.


• One - three years of related work experience demonstrating broad knowledge of the job responsibilities and methods.
