REHABILITATION PROGRAM MANAGER

 Come explore the exciting things that are happening at Pathways, a not-for-profit home health, hospice and private duty provider in the San Francisco Bay Area. We have an exciting opportunity for a dedicated healthcare professional to manage our home health rehabilitation team and programs.

Reporting to the Director of Home Health, you will provide direct supervision to the rehabilitation staff (PT’s, PTA’s, OT’s, OTA’s and SLP’s), coordinate with the operations management team and other programs/departments, maintain appropriate staffing, develop rehab policies and procedures and research/implement new protocols for rehabilitation therapy services.

The ideal candidate will be a CA-licensed Physical Therapist with 2+ years of related PT Home Health experience and demonstrated knowledge of Medicare Home Health regulations and reimbursement issues. OASIS certification is required within 6 months of hire.
