The Job: Physical therapist with Total Longterm Care (TLC) at the Chambers Center in Aurora, Colo. The primary responsibilities of this full-time job include providing clinical care at acute, restorative, and maintenance levels; and integrating physical therapy treatment plans into the team Plan of Care (POC) for maximum holistic gains. TLC is a CMS-designated Medicare/Medicaid program, as well as a certified Program for All-inclusive Care of the Elderly (PACE). Continuity of care ­ Develop one-on-one relationships with your patients, and follow them through their continuum of care from injury to wellness. Types of patients and injuries you would work with: Treat geriatric patients recovering from falls, and restore mobility following orthopedic, neurological, and cardiopulmonary issues.

What you would be paid: A minimum of $45,600 - salary commensurate with experience.
